The challlenge

When Progress Notes Get Written From Memory

Progress notes are a crucial part of the daily work in a skilled nursing facility (SNF):

  1. They give the whole clinical team a clear and granular picture of each resident's condition, so care can be adjusted appropriately.
  2. They serve as the official legal record for the provided care.
  3. CMS reviews them to check whether Medicare Part A claims are medically necessary.

Despite their importance, progress notes are a burden in most SNFs. Nurses spend their shifts with residents, then sit down to document after shift handoff, writing notes from memory. As they write everything at once, the progress notes are short, lack detail and the pressure to finish before the shift officially ends comes on top.

Jason Dugenio, CIO at Bridgeway Senior Healthcare, describes the challenge in the webinar: “Our nurses were documenting at the end of the shift, from memory, at a workstation that pulled them away from residents. The note gets written hours after the care happens, and every one of those hours costs you accuracy and detail.”

The problem with this approach: The time gap between care and documentation costs accuracy and completeness, meaning they also couldn’t always fulfill their actual purpose. Like most operators, Bridgeway had treated the end-of-shift note as the cost of doing business. Until they found voize.

The solution

Documenting at the Bedside, Not After the Fact

It’s a complete, defensible picture of how that resident was managed, and it's exactly what the care team needs to decide whether the plan has to change.”

The collaboration

Side by Side From Day One

Go-live at Bridgeway started weeks before June 1st, with early buy-in from the clinical team led by the Director of Nursing. “I've watched good technology fail because it arrived on the floor as an IT decision. This one arrived as a nursing decision. The DON led it, the nurses saw it weeks before go-live, and some started using it before onboarding was finished. That's what buy-in actually looks like", as Jason says.

The rollout stayed narrow by design: three workflows, 15 nurses trained across the first few days, and the voize team on the floor for the entire first week. Some nurses started using voize on their own before formal onboarding had even wrapped up.

Fabio Schmidberger, voize CEO, underlines the importance of being close to the team on the floor: “We've done hundreds of successful implementations, so we know what successful change management requires.
